COVID-19 Screening Test

Background

Caused by the newly-discovered coronavirus, SARS-CoV-2, the Coronavirus disease (COVID-19) is an infectious disease that has significantly impacted the world today. If you have been experiencing COVID-19 symptoms or would like to get a health certificate required for traveling, it is best for you to get a COVID-19 screening test as soon as possible.

The examination carried out for COVID-19 screening, currently includes but is not limited to a PCR test or a rapid test (the “Test”) using samples taken from individuals participating in the Test. The sample will be taken by the our testing service providers (“Partner”) in accordance with proper medical procedures and in compliance with relevant medical regulations.

Known side effects, reactive antibody test results and follow-up information, if any (collectively, the “Information”) shall be, as far as practicable, communicated by the Partner to the individual who has undergone the Test. All Information shall be treated confidentially by the Partner and the medical staff performing the test.

By purchasing, taking part, or using the Test and/or its results, you acknowledge that you have read, understood, and agreed to all the terms and conditions below:

1. You hereby declare and provide consent for Traveloka Group Companies ("Traveloka", "us", "we", or "our") to collect and process your personal data (including medical data, pre-existing conditions, and other personal data that may be considered sensitive under the applicable laws and regulations) for purposes of conducting the Test.
2. You hereby declare that you understand and acknowledge all consequences of the Test as well as its results. Particularly, if the Test reveals a positive result, you agree to follow any applicable instructions, medical treatments and/or protocols from the relevant medical or government institutions, as the case may be. In this connection, you hereby hold Traveloka free and harmless from any liability that is caused by or related to the Test, its results and/or your non-compliance to the applicable instructions, medical treatments and/or protocols.
3. You acknowledge and understand the risks or possibility of risks associated with the Test, and fully agree that you undertake the Test on your own volition and free will, without any representation or warranty made by Traveloka.
4. If you are ordering the Test for another person, You hereby warrant that you have obtained the required approval from the third party customer regarding the Test and our Terms of Use and Privacy Policy.
5. We are not a health services provider or laboratory, nor we do provide any medical tests, or equipment; hence, we are not responsible for any unintended or adverse result or information relating to the Services carried out by our Partners.
