If you are here for DOCTOR'S VISIT , we strictly request the 2 documents below : a) Interstate travel letter issued by the police ( with relevant travel dates ) b) Hospital appointment letter or card ( with relevant appointment dates ) 5. If you are here for PERSONAL REASONS, we strictly request the 1 document below : Interstate travel letter issued by the police and the reasons stated in the document SHOULD INCLUDE : a ) your check-in and check-out date b) Reason of travelling c) Name of our hotel You may be barred from check-in and will requested to cancel your booking online if you failed to provide these approval letters.
