According to the most updated regulations, only citizens of these following 19 countries are allowed to enter Indonesia on tourist/limited stay visa, including but not limited to: Saudi Arabia, Uni Arab Emirates, New Zealand, Kuwait, Bahrain, Qatar, China, India, Japan, South Korea, Liechtenstein, Italy, France, Portugal, Spain, Sweden, Poland, Hungary, and Norway.
If you do not have diplomatic and official visa holders, diplomatic and official stay permit, holders of Indonesia temporary Residence Card (KITAS), and Indonesia Permanent Resident Card (KITAP), then you have to apply for an e-VISA here. This time, the visa on arrival will not work for you as it is still suspended until further notice. Check the new regulations on the government’s website.

Yes, all international travelers need to quarantine in designated hotels. There are 55 designated hotels on Bali approved by the government. Quarantine for those who are fully vaccinated is only 3 x 24 hours, while the partially vaccinated need to quarantine for 5 x 24 hours.
