How to claim?
Send to PT FWD Life Indonesia
Send claim form and supporting documents to the address mentioned in the footer of the claim form, no later than 90 calendar days from occurrence date for Death & Total Permanent Disability benefit and 30 calendar days from the end of treatment date for Medical Reimbursement benefit.
Payout in 14 working days
PT FWD Insurance Indonesia will disburse the claim payout in maximum 14 working days after all required documents (hardcopy) have been received and approved by FWD. Should there be additional inquiries, you can contact PT FWD Insurance Indonesia at the contact information below.

Who is eligible for this plan?
FWD Life Insurance eligibility is listed below:
- Indonesian citizens.
- For Bebas Rencana, policyholders are between 18 – 55 years old and the Insured are between 18 – 55 years old (last birthday).
- For Bebas Aksi, policyholders are between 18 – 65 years old and the Insured are between 18 – 65 years old (last birthday).
- For both plans, Policyholder must be the same as the Insured.
What is not covered
Your claim will be deemed invalid and the benefit will not be paid if the accident is:
- A result of suicide, attempted suicide, or self-harm; either done consciously or unconsciously.
- A result of involvement in illegal, neglectful, or unlawful acts, including fraud committed by any party with an interest to the insurance coverage.
